Welcome To Our Services
Whether you are experiencing symptoms of Alzheimer’s or another dementia or you are caring for someone who is, life with cognitive loss presents challenges that most of us are unprepared to contend with. Created by former caregivers, the Alzheimer’s Family Support Center (AFSC) is a 501(c)3 independent nonprofit providing a broad array of supports and services to families and individuals throughout the entire disease progression, from early care planning to dementia-specific bereavement support. All our services are provided free of charge regardless of ability to pay. Our goal is to help you build the best support system possible that will serve you over the potentially long duration of dementia disease progression.

Care Planning & Consultations
AFSC Care Consultations provides free, confidential care consultations for families living with Alzheimer’s and other dementia-related diseases.

Social Engagement & Respite Services
Meaningful activities and social engagementprogram for those living with cognitive loss.

Support Groups
The AFSC offers 40+ support groups for caregivers, people living with dementia diseases, and bereavement groups. All groups are currently accessible via Zoom.

Educational Programming
The Savvy Caregiver is a free six-session educational training for active family caregivers of people living with Alzheimer’s or other dementia-related diseases

Cognitive Health Screenings
AFSC offers cognitive health screening across Cape Cod. Screens serve to help ascertain normalcy in age-related cognitive decline versus the need for further clinical assessment.

Community Education
Whether you are living with cognitive decline or caring for someone who is, education is the cornerstone of effective and sustainable Dementia caregiving, and is fundamental to the creation of dementia-friendly community. AFSC provides educational opportunities for professional and family caregivers, first responders, medical professionals and anyone working in the civic and business sectors.
